Export and retrieve form data

Select a dated subset of responses, queue an export, retrieve it from Downloads, and check its contents.

The Form Response Export report pulls submitted responses out as a file. This example exports Daily Visit Note responses by Submitted Date.

You will find it under Reports > Form Responses, and you need permission to view and export those responses. What you see is limited by your access and your filters.

Exported care information is sensitive. Export only what you need, and store and share the file the way your organisation requires.

  1. Step 1

    Open the report

    Go to Reports > Form Responses. If responses or the export action are missing, check your access before concluding there are no records.

  2. Step 2

    Choose the form and dates

    Choose Daily Visit Note under Form Type. Set Filter Date By to Submitted Date, then set the range — the quick buttons (Last 7 / 30 / 90 Days, Last Week) are faster than typing two dates, and the list reloads to the responses inside it. A different date basis selects a different set of responses, so make sure this one matches your review.

  3. Step 3

    Check the selection

    The line above the list counts what you have: so many subjects and so many responses selected, out of how many are available. Select All takes the lot; untick anyone outside this review — a client whose notes another team handles, say — and watch the count come down. What is selected is what is exported. An empty list can mean your filters or your access, not an absence of submissions.

  4. Step 4

    Choose the format

    Choose Excel for a tabular review. PDF and CSV are there where offered. Formats differ in layout and in whether attachments come with them.

  5. Step 5

    Export and wait

    Select Export. Nothing arrives in your browser: the request is queued and the file is built in the background. It appears at the top of Downloads straight away, and stays queued until the generator reaches it. Wait for that row to read Completed, then Download. If it fails or stalls, read its status rather than queuing it again.

  6. Step 6

    Check the file

    Collect it while it is there — downloads do not stay forever. Open the file and check a response you expect to see, its date, and the fields you need. A successful but empty export is not proof there are no records. Store and share it through your approved process.

    Other ways in

    PDF and CSV are alternatives to Excel, and the separate-files option produces one file per subject zipped together rather than a single sheet. The Browser view under Forms answers smaller questions without an export at all.
